Heartland Whole Health Connect aims to create a scalable, evidence-based model for rural health transformation by:
Expanding access to primary and preventive care
Strengthening chronic disease management
Increasing telehealth and remote care utilization
Training providers in whole-person care approaches
Measuring outcomes and sharing learnings across the country
Over five years, the initiative will track patient engagement, provider training, and key health outcomes to help inform rural health solutions nationwide.
Our Partners
Heartland Whole Health Connect brings together six health care organizations committed to expanding access to whole-person care across rural Arkansas. Together, these partners serve approximately one million residents across 30 counties and are collaborating to strengthen care delivery through innovation, technology, and community-based solutions.
Mercy: Regional fully integrated health system leveraging expansive virtual care infrastructure.
Community Clinic: A community health system delivering accessible primary care with integrated dental and behavioral health services to communities across Arkansas and rural Oklahoma.
Boston Mountain Rural Health Center: FQHC delivering essential primary care across mountain and rural communities.
North Arkansas Regional Medical Center: A nonprofit health care system dedicated to providing comprehensive hospital, primary, specialty, and obstetrical care for patients across North Central Arkansas.
St. Bernards Healthcare: An independent, nonprofit, Catholic healthcare system serving 23 counties in Northeast Arkansas and Southeast Missouri.
Arkansas Community Pharmacy Enhanced Services Network: Network of clinically integrated community pharmacies providing enhanced medication management and local care coordination.
Together, these organizations are creating a scalable model for rural health transformation that can improve outcomes, strengthen local care networks, and help shape the future of whole-person care nationwide.
